One of those rescued rode the helo like this, and one of the two from the rescue Apache got out and rode outside too. One of those rescued flew inside. It usually takes a Brigade Commander (Colonel) or Division Commander (Two Star Gen.) level of approval to do so in training or combat. When I was in Iraq, i think it only happened once...and that was in the conventional Army forces. I have no idea if Special Forces or Delta ever did it, but its definitely a last resort evac.
